Payment for Co-pay or Self-pay
List of Medications, Allergies, Past Surgeries
Insurance and Billing Information
We accept most Louisiana health care insurances, Medicare and Medicaid.
Cancellation & Appointment Policy
If you do not cancel your appointment at least 24 hours in advance or if you no show, you will automatically be billed a $50.00 charged.
Procedures Covered by Insurance
If you have health insurance, your insurance will be billed. Insurance is verified prior to each office visit. Co-payments are collected at the time of your visit. We will contact your insurance carrier after your visit for any necessary authorizations.
Financial Policy
Payment is expected at time of service. We accept cash, credit and debit. Payment will include any unmet deductible, co-insurance, co-payment amount or non-covered amount from the insurance company. If you do not have insurance, payment is due at time of service.
